Privacy Policy & Waiver Release
At the Omemee Curling Centre, we are committed to collecting, using and disclosing your personal information responsibly and to maintaining the accuracy, confidentiality and security of such personal information.
Personal information includes information that tells us specifically who you are, such as your address, telephone number, e-mail address, age- category and curler history. However, publicly available information, such as a public directory listing of your name, address and telephone number is not considered personal information.
We have adopted the following privacy policy that guides how we collect, use and disclose personal information about our members and prospective members. Our intent is to balance our legitimate business interests in collecting, using and disclosing your personal information against your reasonable expectations of privacy.
Definitions
In this policy:

Collection of Personal Information
Our primary purpose in collection personal information from you and about you is to maintain an accurate membership file. We only collect personal information that we consider necessary for achieving this purpose and to permit us to conduct our Curling Centre business.
Use of Your Personal Information
We may use the personal information we collect from and about you to:

We may use personal information about you, in a non-personally identifiable manner, to improve our marketing and promotional efforts, to analyze membership, to customize our web site content and layout and to comply with membership affiliation in the Ontario Curling Association.
Disclosure of Personal Information
As a matter of policy, we do not sell, rent or lease any of your personal information to third parties without your explicit consent. We may disclose your personal information as may be required by law or to your legal representative if we are satisfied that the individual is requesting the information on your behalf.
You may access, review and/or change the personal information we have in our files by submitting a written request to our Privacy Information Officer. We ask that you promptly advise us of any changes to your information.
Upon your written request, we will destroy, as soon as reasonably possible and in accordance with our internal policies and applicable law, any and all personal information that we have in our files about you. Notwithstanding the foregoing, we will retain in our files some personal information in order to enable us to collect any unpaid accounts and/or as may be required by law.
Other Information Collectors
Except as otherwise expressly included in the Privacy Policy, this document only addresses the collection, use and disclosure of personal information we collect from and about you. To the extent that you disclose your personal information to other parties, different rules may apply to their collection, use or disclosure of such personal information. Since we do not control the privacy policies of third parties, you are subject to the privacy policies of such third parties.
Protecting Your Privacy
The personal information that is retained by us is kept in a secure environment and we use reasonable procedural and technical safeguards to protect your personal information against loss, theft and unauthorized access or disclosure.
Amendments to This Policy
We may amend this Privacy Policy at any time by mailing and/or posting the amended items on the web site and/or at the Club. All amended terms shall automatically be effective five days after mailing or posting.

RELEASE AND WAIVER OF LIABILITY
I, hereby acknowledge that I am aware that there is the potential to suffer injuries while curling, particularly from falling on the ice, and I further acknowledge that I am aware that if I have not curled before then I should receive instructions before attempting to curl. I am voluntarily requesting that I be accepted as a member of the Omemee Curling Centre and be allowed to participate in the sport of curling and have use of the Centre’s premises at 15 James St Omemee. In consideration of the Omemee Curling Centre accepting me as a member and allowing me to participate in curling, I, on behalf of myself, heirs, personal representatives, successors and assigns, hereby forever agree to release, discharge and undertake not to sue the Omemee Curling and Recreation Club inc. or its directors, employees or contractors from any and all claims, demands, causes or action or liability of any kind whatsoever for injuries, property damages or death that may in any way result from or arise out of my participation in the Omemee Curling Centre including, but not limited to, injuries suffered on the ice while curling or injuries arising out of my use of the Omemee Curling Centre equipment or facilities. I acknowledge that I have read and voluntarily signed this release and waiver of liability and indemnity agreement, and further agree that no oral representations, statements or inducements apart from the foregoing written agreement have been made.
Member’s Consent: I have read and understood the Privacy Policy; and Release and Waiver of the Omemee Curling & Recreation Club and I hereby consent to the collection and use of my personal information in accordance with the Policy. Also, in registering for membership I agree to be governed by the by-laws of the Omemee Curling and Recreation Club.
